About the memorial: Warrant Officer O'Donnell received his first George Medal for disabling a rocket bomb while under fire in Basra in 2006. His second, posthumous GM was awarded after he was killed in Helmand Province, having defused over 50 IEDs before one killed him. This was the first bar awarded to a George Medal for 26 years. The Pre-application enquiry form is a series of questions to see if your project is eligible. If it is, you will need to provide further details and submit current colour photographs of the war memorial in either a png, gif, jpg or jpeg format.
